From a medical professionals viewpoint, a community outreach, event based practice setting offers a unique environment centered on patient care, interdisciplinary teamwork and collaboration. From a patient perspective, each community event based setting is organized to function similar to that of a doctor's office. You will see patients within their local neighborhoods, typically within 1-3 hours from your home.
Community event-based settings offer patients convenience and accessibility to select Health and Wellness Medicare services in their local community where regular doctor's offices may not be as accessible for simple, but needed Annual Wellness Visits. This setting also offers care coordination as we provide all patient results to their physicians so they may optimize their patients health. When needed medical appropriate followup with patients physicians is coordinated as well.
Each location is staffed with state of the art medical equipment, lap-tops and trained medical professionals including a Licensed Nurse Practitioner with access to a Collaborating Board Certified Physician, a Certified Medical Assistant and Customer Care Coordinator. Locations are structured to maintain privacy using partitions to create a registration/ waiting area, exam room and private patient/provider consultative areas.
Locations for these settings may include a country club, recreation center, a local church, a partnered hospital or other common public facility. Locations are retained well in advance and services are provide at locations Monday - Saturday, during day shift hours of 8am - 4pm. Services at any location are not available on Holidays.
Life Line Community Healthcare's medical teams share locations with our sister company Life Line Screening's medical teams. Life Line Screening is not a Medicare provider, but provides direct to consumer preventive health screening services to adults 50 and older. Both teams however support a joint purpose for prevention and service a similar demographic.
All team members on both teams will be centrally located within one to three, sometimes four hours of scheduled community events. By company vehicle, team members may commute together or sometimes individually to events. Event schedules are accessible online for all team members to view 30 days prior, allowing teams to know and plan their schedule.
