Community Outreach Approach
Life Line Community Healthcare takes a community outreach approach when it comes to servicing eligible Medicare beneficiaries seeking to maintain and improve their health. Reaching out to local community sites allows us to give patients access to a provider in a comfortable well-known environment while still coordinating with their local physicians. Taking this approach means we reach out to local communities throughout the United States to establish one or more days a year in their area to provide select health and wellness services to Medicare patients. This also allows us to act as a support tool for Primary Care Providers as we help identify gaps in preventive care and provide necessary information needed to help optimize their patient's long-term health.
Our community outreach teams practice medical professionalism and collaboration while providing quality service and ongoing care coordination. Each team has portable state of the art equipment and consist of a Licensed Nurse Practitioner, Collaborating Board Certified Physician, Certified Medical Assistant and Customer Care Coordinator. Teams are often on location with Life Line Screening teams to provide comprehensive health services and advocate medical appropriate follow-up with local care physicians.
